I can't think of what to call what I'm searching for....

    Did you ever know what you're trying to do, but can't quite figure out the search term to find it? That's what I'm stuck on.

    I'm trying to create a web-based app that will take questions and generate responses based on the answer. For example:

    What kind of family law issue are you having?
    a) Divorce
    b) Separation
    c) Custody and Access
    d) Maintenance and Alimony

    Then, based on the response, generate a series of fields to be filled out, and based on those responses potentially generate more questions and answers, which would hopefully collect all the relevant required data.

    Any thoughts on what the term is I'm trying to look for? I was trying things like multipath form creation tool, etc, but just wasn't quite getting what I was looking for. Google forms looks like it might be promising, depending on what I can do based on the above example - it seems more survey-based at first blush.

    Amyhow, I know some of you are high-tech web gods, so any input would be appreciated. We really don't have the funding to start a new web project at the moment and are looking to use our internal resources (me) to complete it.

    LOL. I saw the post on my phone and was thinking, "Mnemonic? Never heard of it".

    The word(s) I was looking for were conditional questionnaire or conditional logic questionnaire. Now to see what's out there. Thread recaptcha!

    I'm looking at Ninja forms addons for two pieces of the puzzle. No coding required and I may be able to get the specific PDF output I'm looking for. (ie - perfectly resembling an existing print document).
